Aujourd’hui, le Prof. Paolo Lanzetta nous a présenté un cas pour le moins inhabituel.
Tout commence par la découverte d’une hématochézie chez un nouveau-né de 20 jours, ce qui conduit à la réalisation d’un bilan ophtalmologique. Le fond d’œil révèle alors une image frappante : une coloration crémeuse, rose pâle à jaunâtre, des vaisseaux rétiniens, touchant à la fois le pôle postérieur et la périphérie.
Un tableau typique de lipémie rétinienne (lipaemia retinalis), une manifestation rare mais très évocatrice d’une élévation massive des triglycérides. Le prélèvement sanguin du nourrisson en apporte d’ailleurs la confirmation immédiate : un sérum d’aspect laiteux, impossible à analyser tant les lipides saturent l’échantillon.
La prise en charge a reposé sur un échange plasmatique, permettant une amélioration rapide de l’aspect sanguin et une normalisation progressive du fond d’œil, avant d’instaurer un traitement de la cause sous-jacente.
Même si elle est rare, cette présentation peut également survenir chez l’adulte. Dans ce contexte, il s’agit d’un signal d’alarme qui doit conduire à un bilan lipidique urgent, afin d’éviter des complications potentiellement graves à long terme.(Hinkle et al. 2018; Wang et al. 2020; Wetzel et al. 2021)
Un cas impressionnant, qui rappelle combien l’examen du fond d’œil peut parfois offrir une fenêtre unique sur des désordres métaboliques sévères.
